Interproximal reduction, often called IPR, is a planned orthodontic procedure that removes a small amount of enamel from between selected teeth. The goal is usually to create a modest amount of space, improve the fit or proportions of the teeth, or help an orthodontist guide tooth movement. It is sometimes called slenderizing, reproximation, stripping, or selective reduction.
IPR is not the same as filing your teeth at home, treating a cavity, or removing an entire tooth. It is a controlled part of an orthodontic plan. Whether it is appropriate depends on the teeth, the amount of enamel available, the alignment goal, and the other ways space could be created.
Why would an orthodontist recommend IPR?
Teeth can be crowded even when the amount of crowding is relatively modest. In selected cases, making a little room between teeth lets the orthodontist align them without relying on a larger movement or another space-creating approach. IPR may also help address a difference in the relative widths of upper and lower teeth or refine the shape of teeth as they are aligned. With clear aligners or braces, the space is then used as part of the larger movement plan; the enamel reduction itself does not straighten a tooth.
The amount and location matter. An orthodontist may recommend reduction between particular contacts rather than treating every gap in the mouth. The purpose should be understandable: ask which teeth are involved, how the space will be used, and what alternatives were considered. A recommendation is not a statement that IPR is necessary for every person with crowding.
What happens during the procedure?
The orthodontist first evaluates the contact between the teeth, their shape, the planned movement, and the condition of the enamel and gums. A thin abrasive strip, disc, or another dental instrument may be used to reshape the contact gradually. The clinician then smooths and polishes the area so the new contact is comfortable and cleanable. The exact instrument and sequence vary by treatment plan.
People may notice pressure, vibration, or a scraping sensation. Temporary sensitivity is possible, but not everyone experiences it. The procedure is designed to remove only a planned amount of enamel; enamel does not grow back, which is why the decision and measurement belong to the treating dental professional. Do not try to reproduce IPR with a nail file, abrasive strip, or other home tool.
Does IPR damage teeth or cause cavities?
The concern is reasonable because enamel is the hard outer layer of a tooth. Reviews of clinical studies have not found a clear increase in caries, periodontal changes, or sensitivity in the studied IPR groups, and one review found caries occurrence statistically similar between treated and untreated surfaces over follow-up periods ranging from one to seven years. However, the evidence has important limits: the studies were not all high quality, methods and polishing protocols differed, and long-term evidence is less complete than the confident-sounding internet claims suggest.
That means the most accurate answer is neither “IPR is dangerous” nor “IPR is risk-free.” The clinician must choose an appropriate amount, leave the surfaces smooth, and account for oral hygiene and the condition of the teeth. A rough or difficult-to-clean area, ongoing decay risk, or unexplained sensitivity should be assessed rather than dismissed. Your orthodontist can explain how those factors affect your plan.
What should you ask before agreeing to it?
Useful questions include:
- Which teeth will be adjusted, and what problem is the adjustment meant to solve?
- How much enamel is planned at each contact?
- How will the surfaces be smoothed and checked afterward?
- What other ways of creating or managing space are relevant in my plan?
- Could the plan change if the teeth or gums are not healthy enough for the intended movement?
These questions do not require you to choose a treatment yourself. They help you understand how a small enamel adjustment fits into the overall orthodontic sequence. If the procedure has already been performed and you have persistent sensitivity, a sharp area, pain, or a concern that a tooth was altered unexpectedly, contact the treating practice for an examination.
IPR is best understood as a limited, deliberate space-management tool. It can be useful in the right orthodontic plan, but the decision depends on individual anatomy and treatment goals. The safest explanation is the one that names both sides: the procedure is conservative in concept, and the enamel removed is permanent.
